A 71-year-old lady attended ENT clinic with a nine-month history of right pulsatile tinnitus and peri-nasal pain. There was no hearing loss, vertigo or nasal complaint. Otoscopic and flexible nasendoscopy examination was entirely normal. On auscultation there was no neck bruit.
